Transitional Family Homes

For youth with emotional and/or behavioral disturbances, Therapeutic Foster Homes provide community-based services within a family home setting. The  3-9  month program is often a “step-down” from hospitalization or a residential setting and can be helpful in transitioning youth from a higher level of care to successful integration into the community.

The Program strives to help youth and families reunify when possible. If not, efforts are focused on supporting the youth’s transition to a long-term placement in long-term foster care or with relatives.

Foster parents that provide Therapeutic Services receive pre-service training as well as ongoing monthly trainings and support. In addition, a Qualified delete qualified Mental Health Professional provides weekly supervision to assist foster parents in providing a therapeutic family environment for youth.

Population

  • Youth ages 9-17 in need of a therapeutic home environment

Admission Criteria

  • Youth requires out-of-home placement
  • Best served by receiving mental health services in natural family setting
  • Must agree to participate
  • Must be referred by Department of Health and Family Guidance Center

Youth Responsibilities

  • Participate in program activities such as individual and group sessions
  • Participate in creation of treatment plan and work toward treatment goals
  • Abide by program and foster home rules and expectations
  • Attend school

Discharge Criteria

  • Youth achieves goals and completes program
  • Youth runs away
  • Youth is removed by referring agency
  • Youth requires higher level of care
